COACHES REGISTRATION REQUIREMENTS

A Travel Team coach needs, at a minimum, an F License before he/she can coach. In addition, the State (NJYS) requires a current Kidsafe Disclosure form to be on file, for every year the coach is actively coaching. Please, follow the steps below to prepare and submit all the necessary forms to register as a Travel Coach with FTSC, MNJYSA and NJYS:

Download and complete the following forms: NJYS Coach's Membership Form eForm NJYS Kidsafe Disclosure Form eForm Get Free Acrobat Reader® software so you can view and print Adobe PDF files. click here	The following must be provided for member coach passes: * Copy of coach's F LICENSE Certificate. NO COACH will be carded and allowed to coach a team unless proof of an F LICENSE is submitted. * 1½" x 1½" size, COLOR photo for Coach's ID Card. (min photo size is 1" x 1". Absolutely NO black & white photos are accepted)
Submit all forms along with Players' paperwork and Team Packets for League Approval.
