for member player passes:
* Copy of player's
Birth Certificate with Date of Birth clearly visible.
A
copy of player's Passport is also acceptable.
* 1½" x
1½" size, COLOR
photo for Player's ID Card. (min photo size is 1"
x 1". Absolutely NO black & white photos
are accepted)

submit completed forms to your team coach or manager
along with a
check payable to "FTSC" in the amount indicated
in the FTSC Registration Form and include the player's
name and team on each check. |
