The preferred method of registration is on-line (see the On-line Registration link on the left side of this page). On-line registration provides the option to pay be credit card or check, and provides an immediate verification that your registration has been received. On-field registration occurs on the last two Saturdays of the season. The forms below are made available for mail-in registration if on-line or on-field registration not possible. The applicant is responsible for the form's completeness and correctness. If problems or errors exist, the applicant will not be registered until the corrections are made. Therefore, on-line or on-field registration is strongly recommended. To use these forms, they must be printed out and mailed in with a check. Fall 2010 Forms:

The Franklin Township Soccer Club Scholarship Program is intended to recognize those individuals who have participated in and/or made contributions to the Franklin Township Soccer Club. This scholarship is offered to college-bound seniors who have participated in the Franklin Township Soccer Club as a player, referee, coach, or assistant coach during their high school years. The Franklin Township Soccer Club will award a monetary award to a graduating senior who is planning to obtain a degree from a four-year program at an accredited college or university. Applications must be postmarked by May 17th, 2010. |

These forms are included in the coaches' packet. The Accident Reprot form is to be completed and filed with the Club following any injury that occurred during a practice or game. The Medical Release form must be completed by parents and returned to their coach to participate in all practices and games. Coaches must have the team's medical release forms available to them at all practices and games. The Trophy Order form should be submitted for each team even if there is no trophies ordered. (Set your printer orientation to landscape rather than portrait to print this document.) Trophy Order Correction Form should be submitted only if there are errors found on any team trophies. The KIDSAFE Letter addresses the requirement of all coaches and assistant coaches to complete the New Jersey Youth Soccer Association KIDSAFE Program. KIDSAFE forms are available at and should be returned to the information desk. The Notice to Coaches covers the responsibilities coaches have at the start and the end of game days.
